ESSENTIAL JOB FUNCTIONS

Manages the Admissions Process
Assists with pre-enrollment tasks to prepare for a client's enrollment
Facilitates the admission intake process.

Manages On-going Treatment
Within a client's first 30 days after enrollment, ensures that the appropriate individual plans (IPOC, CIPOC, IEP, PCP) are established.
Maintains contact with the guardian and agency representatives after the client is enrolled.
Between MDT meetings, coordinates and facilitates all aspects of treatment and care for all clients on the case load.
Attends or facilitates multidisciplinary team meetings to monitor client progress and updates the Plan of Care.
Coordinates, with the treatment team, departure and arrival arrangements for off-site visits with the guardian.
Coordinates and facilitates the new IEP/PCP and revisions to those documents.
Ensures documentation for the client is accurate and properly maintained in the client record.

Manages the Transition or Discharge Process
Facilitates discharge from Grafton.
Facilitates transfer to another Grafton program or level of service provided by Grafton.

Communicates between Grafton and External Stakeholders
Provides on-going communication between Grafton and external parties.
Prepares for and attends FAPT/PCP meetings.
Manages 9 week / quarterly report process.
Responds to stakeholder complaints regarding all aspects of treatment and care for client.

Leadership and Organizational Committment
Demonstrates knowledge of Grafton.

Demonstrates core values through personal behavior.
Communicates effectively by selecting appropriate communication modalities.
Promotes teamwork.
Manages and implements change.
Performs other duties, as assigned.

MINIMUM HIRING REQUIREMENTS

Masters degree in social work, psychology or counseling; or,
Bachelor degree in social work or psychology with at least one year of documented field experience; or,
Bachelor degree with at least three years experience working with children in a residential facility.

Knowledge of CSA/Medicaid funding processes.

Knowledge of services accessible to special needs population
