Assist the Academic Teacher in meeting academic and vocational goals through planned, structured instruction directed by the teacher. Also, provide support to improve skills in the communication, physical abilities, social-emotional domains and self-care domains. May assist with morning routines in the residence before school and provide transportation to school. Work with clients in residences during academic breaks and holidays and during those times assists with household maintenance and meal preparation.

Ensures medications are delivered accurately and on time
Provides for the well-being and safety of the client
Follows infection control procedures
Individual Plan Implementation
Understands and participates in the individual planning process
Uses the individual plan to become informed about the client's strengths, needs, preferences, interests, and accommodations needed.
Implements and promotes clients' achievement of educational and skill development goals
Implements the treatment/ behavior plan
Provides a Support and Solution-Focused Environment
Builds trusting, respectful relationships with clients
Focus is on client progress - improving opportunity and quality of life
Manages behavioral crisis safely
Completes standard documentation
Collaborates with multi-disciplinary team to provide continuity of support and care
Household Management - Housekeeping, Nutrition, Transportation, Finances
Ensures safe residence and school environment
Assists with household chores and assures a clean and tidy home
Assists with nutrition and, in group homes, with meal preparation
Provides transportation / community mobility
Assists with purchases and is accountable for monies
Teamwork and commitment to the mission, values and vision of Grafton
Demonstrates knowledge of organization
Demonstrates core values through personal behavior - integrity and accountability
Communicates appropriately with stakeholders - other employees, supervisors and managers
Minimum Hiring Requirements
Must be 21 years of age
A Bachelor's degree in human services; or an Associate's degree and 3 months experience working with children; or be a high school graduate or have a G.E.D. and 6 months experience working with children
Valid driver's license with an acceptable driving record.
