Duties:
- Completion of initial and annual Physician Referrals for students who require Occupational and/or and Physical Therapy evaluations and/or treatment in the school setting
- Documents significant medical diagnoses/concerns, including appropriate ICD-10 Codes
- Documents precautions and contraindications
- Identifies areas of concern for school-based treatment.
- Consults with other clinicians/related service providers, patients/families, and other school staff concerning treatment when appropriate:
- Refers students to outside treatment facilities, when appropriate, to ensure high-quality evaluation, treatment and continuity of care.
- Maintains student records and ensures privacy according to HIPAA and FERPA regulations.
- Adheres to the Chicago Public School Policies and Illinois State Standards.
- Collaborates with and reports to ODLSS and OSHW management teams to facilitate and devise systems and oversight for effective administrative operation of OT/PT referral at CPS
Minimum Requirements:
- A graduate degree in nursing and/or eligible for licensure as an APRN in Illinois.
- Possession of a current APRN License (Certified Pediatric or Family Nurse Practitioner} in the State of Illinois
- Certification as a nurse practitioner in area of specialty by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP).
- 1 year of experience in community based health settings, school nursing, or pediatric nursing.
- Proficient in pediatric primary care knowledge about OT/PT efficacy and scope of services
- Knowledge of Illinois medical information release procedures and laws.
- Knowledge of HIPAA and FERPA rules, regulations, and compliance.
- Knowledge of OSHA regulations for blood borne pathogens.
- BLS/Healthcare Provider Certification
- Current National Provider Identifier (NPI) number
- Current State of Illinois Medicaid Provider Number
- Strong written and verbal communication skills
- Proficiency with electronic documentation systems
Terms of Employment:
- CPS employees are required to be actual residence of the City of Chicago within six: months of beginning employment, and are required to maintain Chicago residency throughout employment.
