JOB SUMMARY:

The Title I home school coordinator provides support, education, and resource referrals to students and families. The Title I home school coordinator will provide interventions to students

and perform support services to individual students and their families to assist with social and

emotional concerns related to their educational and social progress. The Title I home school coordinator will provide consultation and support to staff.

DUTIES/RESPONSIBILITES:
  1. Provide at risk students with education, support and resource referrals that support educational and social progress.

Examples of Work Performed:
  • Provide individual, family or group support for children who are experiencing problems in school, home, or community with an emphasis on early intervention and prevention.
  • Develop support plans for students with identified goals, and report progress of the student.
  • Provide crisis intervention which is geared towards protecting the safety and emotional well-being of students.
  • Complete home visits with students and families as necessary to improve the home-school connection with the goal of improving academic and behavioral success.
  • Consult with parents, teachers, nurses, counselors, administrators, other school staff, as well as outside agencies, to assist in the planning and programming for students.
  • Set individual student goals, document services provided, and monitor progress of each individual student.
  • Analyze specific data of students who require additional supports to determine needed interventions and progress. (e.g. discipline referrals and attendance)
  • Encourage and assist families in the pursuit of community services that may assist with the current presenting problem.
  • Perform other duties as assigned based on student, teacher, school or District needs.


2. Provide support to teachers and other school staff for students with social and emotional concerns as related to their educational and social progress.

Examples of Work Performed:
  • Receive information from staff, administrators, and parents regarding appropriate services at school, and in the home, for students who require additional support.
  • Attend and participate in staffing, IEP meetings, 504 plans, and care team referrals to provide input in developing treatment goals, behavioral interventions, and additional supports for the child.
  • Provide staff support, identify resources and education in the area of childhood mental health and human behavior.
  • Report progress of students to staff and administrators on an on-going basis.
  • Participate as the leader of the school wide Character Education/PBIS leadership team to provide input in developing behavioral interventions and intervention system support.
  • Develop a network of support systems with social workers, outside agencies, and district staff to support students and families.


3. Attend and participates in all required meetings and professional development.

  • Actively lead and/or participate in district professional development activities.
  • Actively lead and/or participate in building professional development activities.
  • Actively lead and/or participate in specific professional development meetings as deemed necessary to assist students.


MINIMUM QUALIFICATIONS

EDUCATION/CERTIFICATION:

Bachelor's degree in education or related field required, Masters preferred

Missouri DESE certificate in secondary education or school guidance counseling required

REQUIRED KNOWLEDGE: Broad knowledge of restorative circles, student interventions, developing proactive student support plans and support systems for students and families.

EXPERIENCE REQUIRED: Requires experience in providing proactive interventions for scholars based on previous actions, teacher support in building a positive classroom culture, hosting restorative circles, working with faculty and staff developing frameworks for a positive school culture.

TERMS OF EMPLOYMENT: The Title I Home to School coordinator is funded 100% by Title I funds. Should funds no longer be available, or the schoolwide Title I plan change, the position may be eliminated to meet the needs of the Title I school.

SALARY IS BASED ON PLACEMENT OF TEACHER SALARY SCHEDULE-MAXIMUM OF STEP 6 FOR NEW HIRES
