POSITION OVERVIEW:

The Case Management Resource Nurse resides in the Office of Social Emotional Learning and Wellness within the Academic & Student Support for Equity Team of the Boston Public Schools, reporting to the Senior Director of School Health Services. The Case Management Resource Nurse will play an integral role in the standardized implementation of a case management approach through coaching, providing continuous professional development, and developing tools and systems to improve the overall health of students with chronic diseases and advance the mission of the Boston Public Schools. The nurse will possess extensive experience in the management of chronic diseases such as asthma, diabetes and sickle cell disease. The Case Management Resource Nurse will serve in a mid-level role within the Office of Social Emotional Learning and Wellness. The Case Management Resource Nurse will also be responsible for having a high level of expertise with regard to case management which is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the whole child's health and human service needs. It is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes.

RESPONSIBILITIES :
Case Management
  • Work collaboratively with the Case Management Support Nurses to develop case management instructional guides for common chronic diseases (diabetes, asthma, sickle cell disease, seizure disorders, life-threatening allergies)
  • Work collaboratively with the Case Management Support Nurses to assist BPS school nurses with the creation and implementation of individual healthcare plans (IHPs)
  • Create and maintain relationships with community partners directly involved in caring for students with chronic diseases
  • Act as a content expert on management of chronic diseases in the school setting, providing guidance and information to school nurses and SHS central office staff on specific cases
  • Participate in continuous quality improvement projects regarding care management of chronic diseases in the school setting


Professional Development/ Education:
  • Work collaboratively with Nursing Program Director to develop, plan, and facilitate relevant in-person professional development, webinars and educational materials on case management for BPS school nurses
  • Partner with community organizations to engage guest speakers for professional developments
  • Collaborate with Case Management Support Nurses to educate nurses in person or via phone/video chat to provide guidance on the medical treatment/nursing care of students with chronic diseases
  • Support implementation of specific BPS policies & procedures for students with chronic diseases via relevant professional development
  • Keep abreast of knowledge of best practices, state and federal laws, regulations, policies/procedures

Administration:
  • Collaborate with other BPS departments and offices; attend meetings, serve on committees as needed
  • Attend statewide meetings if needed
  • Assist Nursing Program Director and Program Manager in scheduling/coordinating the activities of the Care Management Support Nurses and scheduling of professional development sessions
  • Assist Nursing Program Director and Program Manager with data tracking.; ensure the integrity of data through complete and accurate data entry

QUALIFICATIONS - REQUIRED :
  • Bachelor of Science in Nursing
  • Massachusetts license as a Registered Nurse
  • MA State Dept. of Education School Nurse License (All Levels)
  • Minimum of 5 years of experience in school nursing
  • 3-5 years of experience in leadership and professional development
  • 3-5 years of case management experience, especially with chronic disease case management
  • CPR & First Aid Certification
  • Ability to meet the BPS Standards of Effective Practice
  • Exemplary communication skills
  • Well-versed in Microsoft Office, G Suite and electronic medical records
  • Commitment to self-reflection and ongoing learning
  • Ability to work independently and in a collaborative team environment
  • Comfort with handling multiple projects simultaneously as well as working with rapidly shifting priorities and targets
  • Experience working with culturally-linguistically diverse students, families and communitie s
  • Current authorization to work in the United States
  • An understanding of and commitment to gain a greater understanding of what is necessary for an urban school system to enjoy a continuous improvement in an increasingly complex and competitive environment
  • A deeply held and unyielding belief in the overarching mission of public education
QUALIFICATIONS - PREFERRED :
    • Masters of Science in Nursing

  • Nationally Certified School Nurse
  • Experience in: support services in an urban school setting; liaison and case management; high risk, multi-problem children and adolescents

Terms: BTU, Group I plus 10%.
