We are currently seeking a Social Work Supervisor for one of our Preventative Services programs. Preventative Services is a comprehensive, neighborhood-based program which helps to provide family support and counseling to children and their families. The Family Support Center specializes in providing crisis-intervention services, referrals, advocacy, groups on parenting, and anger management. The goal of this program is to prevent foster care placement by improving the family's ability to communicate effectively and manage the problems that are associated with inner city living.This position serves as an integral role within the team dynamic.

Essential Duties and Responsibilities
  • Participate in weekly supervision with the team
  • Provide supportive casework counseling (individual and family) to families and their children
  • Conduct home visits
  • Conduct home based, office based, and school based assessments of children's health and safety
  • Audit progress notes on FASP, PROMIS and CONNECTIONS
  • Provide toxicology referrals as required and follow-up with results
  • Ensure families are connected to treatment programs and collaborate around service plan and progress
  • Participate in team meetings and case conferences as scheduled
  • Ensure the timely and accurate completion and submission of all required documentation
  • Acquire knowledge of various community based and governmental resources and refer families to and advocate for the acquisition of such resources on behalf of children and their families
  • Evaluate families to determine if they are eligible for preventive services
  • Assess family history, current needs and family perception of service need during the intake process
  • Conduct ongoing assessments of the entire family to ensure that children are safe from imminent danger of harm
  • Conduct on-going family and individual assessments
  • Make referrals for children and families both internally and externally when such referrals are necessary to promote the health and safety of children
  • Participate in all agency functions and project activities
  • Perform other duties assigned

Qualification, Skills and Knowledge Requirements
  • A commitment to the mission and programs of HCZ
  • Master's Degree required; LMSW and significant field work experience preferred
  • Some knowledge and experience with mental health required; must have knowledge base about the effect and impact of substance abuse
  • Must possess some knowledge of child welfare and of ACS
  • Outstanding interpersonal and communications skills, both oral and written
  • Passion for children and families and a strong connection to the community
  • Bilingual is a plus
