WORK SCHEDULE: 188 days/ 7.5 hrs - Union UFCW 1459 contract
BRIEF JOB DESCRIPTION Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's comprehensive health needs. The School Nurse Case manager will work in collaboration and continuous partnership with chronically ill or "high-risk" students and their family/caregiver(s), clinic/hospital/specialty providers and staff, Springfield Public School staff, and community resources to create a team approach in able to: Promote timely access to appropriate care; Increase utilization of preventative and tertiary care; Reduce school dismissals, emergency room utilization, and/or absences; Promote adherence to developed plans of care; Increase continuity of care by managing relationships with tertiary care providers and referrals; Connect patients to relevant resources with the goal of enhancing student health and well-being; Foster school connectedness and academic achievement. Success in this position will lead to improved health outcomes and academic success for the managed student population.
PRIMARY FUNCTIONS &
KEY RESPONSIBILITIES OF THE JOB:
Identify "high-risk" students (the chronically ill, chronically absent (greater than 18days), those with special health care needs) and create a caseload (or flag in EHR) for direct management
Assess and evaluate the caseload of students' medical history, including social, behavioral, psychological and familial concerns.
Participates in direct and indirect nursing care, health maintenance, teaching, counseling, collaborative planning and rehabilitation.
Serve as a liaison and contact person between school nurses, family, community, and health care providers.
Provide school nurses with support on treatment planning and education.
Proactively act as advocate, responding with empathy and respect to resolve to the student's and family concerns and recognize opportunities for improvement to meet health concerns
Participate as the health expert for Individualized Education Plan (IEP) and 504 plans
Identify student needs through a collaborative approach to plan for appropriate evidence-based interventions in the school setting. Planning includes:
Assessment of student's unmet health and social needs
Incorporating the student's goals and related activities into a student-centered plan of care—the Individualized Healthcare Plan (IHP)
Supporting student academic success and addressing family needs, concerns, and preferences.
Aid in development of this IHP with the student, family/caregiver(s) and providers (emergency plan, health management plan, and ongoing action plan, as appropriate)
Monitor adherence to care plans, 504's, IEPs and the students progress in a timely manner
Monitor the application of school nursing care ensuring a high quality of care is being given
Evaluate and facilitate changes to revise plan(s) as necessary.
Create ongoing processes for patient and family/caregiver(s) to determine and request the level of care coordination support they desire at any given point in time
Evaluation of the outcomes
Attend/participate/facilitate home visits, discharge meetings, attendance of medical appointments, and school-based meetings in order to carry out the coordination of plans
Conduct yearly Quality Improvement (QI) initiatives for departmental improvement opportunities
Prepare and submit yearly written reports with data on outcome evaluations for supervisor
Provide health office coverage and additional task/assignments as delegated by Nursing administration.

MINIMUM JOB
QUALIFICATIONS:
Professional Licensure required as a School Nurse
RN with greater than 5-years' school nursing experience
Current CPR and First Aid certification
Current CPR Instructor
BSN degree required; Master's degree preferred
