Transition Program Coordinator

The Transition Program Coordinator assists staff in providing special education and transition services that are critical to the success of students with disabilities by providing guidance and training in a manner that is compliant with all pertinent laws and regulations. Services are provided for all levels of transition planning including, Middle School into High School and High School into the workforce or post-secondary training/education. The position also assists in the completion of related reports that may be required by the school, State and/or federal government and facilitates communications between parents, staff/school administration and third party agencies.
AS Transition Program Coordinator, YOU WILL..
  • Serves as consultant and in-service leader to teachers in the coordination of transition activities; Assists in preparing for/conducting needed in-service trainings including special education orientation;
  • Remains abreast of current research and special education laws as well as attending professional development events in order to act as a resource to Special Education Teachers disseminating knowledge gained and soliciting their feedback in order to identify additional training opportunities;
  • Coordinates transition services for special education students;
  • Communicates/educates/responds to questions from administrators, teachers, parents and outside agencies regarding transition services available for special education students; Assists in their appropriate utilization; Serves as liaison to vocational/transition agencies for the district; Attends agency meetings as school representative.
  • Oversees all interagency linkages for special education transitions;
  • Plans and carries out transition related outings, such as transition tours, job shadows, and job experiences, including meetings with employers and secondary education institutions;
  • Supervises the writing, implementation, and progress of IEP transition plans;
  • Conducts parent, student, and teacher training related to transition topics;
  • Collects and maintains data related to: IEP attendance; survey completion; entry into State database for Indicator 14 survey;
  • Develops course of study for all students with collaboration from counselors;
  • Writes Summary of Performance for exiting students;
  • Creates comprehensive post-high school program for students with significant disabilities;
  • Attends annual transition plan meeting.

Requirements
Minimum Qualifications
  • Bachelor's degree in Education AND
  • Three (3) years of Special Education classroom experience OR
  • Equivalent combination of education and experience
Certificates and Licenses: Special Education Other Required Qualifications
  • Proficiency in MS Office (Word, Excel, Outlook, etc.)
  • Ability to travel up to 10% of time as needed for meetings, professional development, etc.
  • Able to pass background check requirements
Desired Qualifications
  • Transition-to-Work or Secondary Transition Endorsement; Career Technical Education Certification/Endorsement
