In 2005, as a direct response to the public health crisis of obesity in our youth, HCZ created an effort called the Healthy Living Initiative (HLI) to encourage our students and their families to incorporate healthy food choices and physical activity into their daily lives. In the years since then, we have sought to deepen our work in this area and have developed a strategic program model and growth plan called Healthy Harlem (HH), which consists of best practices in physical activity, nutrition education, and community support. Like HLI, this program is designed to instill a culture of healthy living throughout the Zone but is more expansive and strategically focused in the way we reach youth in our programs, their parents and care givers, and the community at-large.

Healthy Harlem is seeking a dynamic, energetic Health Coordinator to join our team. The Health Coordinator will be responsible for managing and administering Healthy Harlem program services, including organizing health fairs, facilitating wellness workshops for parents, developing asthma education materials, and linking participants to health resources. The Health Coordinator will also act as a liaison between Healthy Harlem and its community partners. This position reports to the Community Pride Program Director.

Essential Duties and Responsibilities
  • Organize and facilitate health workshops for HCZ families and community members
  • Establish and maintain collaborations with relevant community organizations and medical providers
  • Lead in the planning of HCZ's annual wellness expo and other special health-related events
  • Connect and refer high-risk participants to wellness services
  • Develop and disseminate asthma education materials to program sites
  • Coordinate Weight Watchers meetings for staff, caregivers, and community residents
  • Engage and train site staff in healthy eating/active living practices
  • Meet with Healthy Harlem Core Team on a regular basis for feedback and to discuss strategies and ways to improve the delivery of health programming
  • Perform other duties as assigned

Qualification, Skills and Knowledge Requirements
  • Bachelor's degree in Nutrition, Community Health Education, or a related field; ideal candidate will have a Master's degree in one of these fields
  • Must be knowledgeable about a wide-array of health topics, including nutrition, physical fitness, asthma, and chronic diseases
  • Familiar with the emotional, social, and mental aspects of a healthy lifestyle, including the safety and overall well-being of individuals engaged in healthy eating/active living programs
  • Comfortable working in various urban settings and with diverse populations
  • Awareness of and sensitivity to cultural and socioeconomic characteristics of populations served
  • Strong organizational and collaborative skills, and ability to work on a team and individually
  • Adapts well to changes in assignment and priorities with the ability to respond effectively to new information, changing conditions, and unexpected obstacles
  • Comfortable working with outside community organizations and contracted program providers
  • Willingness to model Healthy Harlem standards of nutrition and activity and personally demonstrating the caliber of performance and behavior that is expected of the staff and students
  • Must be able to work flexible hours, including some weekends, and perform extensive field work
  • Excellent verbal and written communication skills
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and Outlook
