Intensive Case Manager Role

My Brother's Keeper (MBK) is a Federal & State initiative aimed to address persistent opportunity gaps faced by boys and young men of color and to ensure that all young people can reach their full potential. In Schenectady one of the key MBK goals is to enable all young men of color to graduate from high school college and career ready. Another unique system of support in SCSD is The General Education Continuum which offers a range of programs and tiered supports for students who do not qualify for special education services, but still require intensive supports to reach their fullest potential. The Intensive Case Manager will maintain a strong home, school and community connection. The Intensive Case Manager will work within a diverse environment of dynamic learners. This position will require a high degree of creativity, cultural awareness and problem-solving capabilities.

Intensive Case Manager Expectations

  • Will provide intensive supports for caseload of 15-20 students (seniors) who are in need of course credit and passing regents exams in order to graduate;
  • Collaborate with students, parents, and staff to identify specific barriers to student's success and create individualized academic support plans for each student on caseload that will reduce barriers and ensure success;
  • Provide daily support to students to ensure they are meeting the goals of their academic support plan;
  • Provide & schedule after school academic tutoring to MBK students as needed;
  • Ensure regular communication with families and other stakeholders (MBK Program Specialist & Director of Student Services) about student progress, adjust plans as needed;
  • Assist MBK students with goal setting & organization skills;
  • Serve as an advocate for MBK students & maintain regular contact with their teachers to facilitate success;
  • Provide direct support and guidance around college and career planning for student;
  • Assist students in accessing other community supports in order to reduce barriers to academic success;
  • Analyze student data and make strategic decisions around increasing student success;
  • Collaborate with other key stakeholders to ensure academic support plan is appropriate and ensuring student success (such as content teachers, behavior specialists, homeless liaison, coach, or clinical provider);
  • Position reports to Building Principal & The Office of Pupil Personnel Services.

Intensive Case Manager Key Qualities
  • Dedication and understanding of boys and young men of color;
  • Exceptional interpersonal skills and communication skills;
  • Takes an advocacy approach to coaching and mentoring;
  • Strives to motivate in a team-oriented, collaborative environment.

Minimum Qualifications:

1. Master's Degree;

2. Must hold NYSED Social School Worker or School Counselor Certification in NYS;

3. Experience in urban settings;

4. Experience working with boys and young men of color;

5. Hiring Period: ASAP-August 30, 2019.

Compensation:

  • Grant Funded: Continued employment contingent upon funding.
