Job Description:

The Care Manager is ultimately responsible for the overall provision and coordination of services to assigned caseload. The Care Manager guides program enrollees and their caretakers (legal guardians) through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The Care Manager is responsible for assisting the family in identifying its strengths and needs, while also providing counseling and crisis intervention services youth and families. Specific responsibilities include but are not limited to: Obtaining required consents from the individual or legal guardian, assessing the family's needs as a precursor to service plan development. Completing initial and ongoing needs assessments to determine the most appropriate level of care management, developing a service plan with the family and an interagency team; including but not necessarily limited to goal setting with the child and family and discharge planning, responsible for the overall management of the patient's Individualized Plan of Care, promoting wellness and prevention by linking child and family to all required and needed services as identified in the service plan and based on their individual needs and preferences and identifying available community-based resources and actively manage appropriate referrals, access, engagement, follow-up and coordination of services.

Job Qualifications:

Minimum of an Associate's Degree with two years' experience, Bachelors of Arts or Science or Registered Nurse or Licensed Practical Nurse with two years' experience preferred. Must have experience providing service coordination and information, linkages, and referrals for community-based services and relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare, and/or other systems, as well as those receiving preventive services. Must demonstrate understanding of the needs of high acuity children and their families as evidenced by additional years of experience, education, and training. Valid driver's license and a registered operating car to be used for daily transportation for work related purposes. Bilingual preferred.
