Job Description

SUMMARY: Reports directly to the Students in Transition Coordinator. Provide services to the children and youth in the Students in Transition Program to help them to overcome obstacles and barriers to receiving an education and to assist families with social services needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Understand all federal guidelines as they pertain to the eligibility of children and provision of services for the McKinney-Vento Homeless and Foster Care Program.
  • Coordinate with designees, social workers and school staff in the district to assist homeless and foster care students with obtaining and providing documents and other pertinent materials necessary to enter school and to benefit families.
  • Performs casework service with individual students to correct those personal, social, or emotional maladjustment's related to their educational and social progress.
  • Assist and provide direct service to unaccompanied youth, mentor and provide supplies, resources and case management in an effort to support them toward high school graduation and utilize youth services, runaway and HUD programs for housing.
  • Performs casework service with parents to increase the parents' understanding, their constructive participation in resolving their child's problems, and their knowledge arid use of appropriate resources available.
  • Utilize crisis intervention skills, case management and community resources knowledge to support the family to move toward greater family stabilization for better school performance when possible.
  • Monitor attendance and truancy of homeless and foster care students.
  • Assist with medical needs i.e., glasses, immunizations, dental and mental health that impairs school performance.
  • Check on enrollment records, and identify support needs. Assist with uniforms and clothing. Supply parent rights, back packs, so that student socially blends in with other students in their school.
  • Assist with placing children in tutoring, mentoring or after school programs.
  • Assist parents in motels or doubling up with referrals for food, clothing, and shelter.
  • Network with community agencies to provide appropriate referrals to agencies, organizations, housing and health care.
  • Assist families with resources and services to overcome homelessness.
  • Provide the shelters with educational resources and information regarding school district resources.
  • Supervises the referral of students and families to, and serves as liaison with, such outside agencies as the Welfare Department, Child Placement Bureau, and Juvenile Court, as appropriate.as designated by law and school board policy.
  • Makes home visits for the purpose of gathering helpful information on a student's background.
  • Serves as liaison between home and school when considerable follow-up is necessary, as in welfare cases, foster-home children, and disadvantaged students.
    • Keeps sufficient records of cases for use by school staff members arid outside agencies when appropriate.
  • Performs other duties assigned.

  • Bachelor of Science or Bachelor of Arts Degree with an emphasis on social work or counseling and a minimum of two years' experience in the field.
  • Successful completion of a background check by Human Resources.
