Lake Forest Community High School is seeking a full-time Long-Term Substitute Transition Program Teacher from the anticipated date of April 12, 2019 through June 7, 2019.

Description: The Transition Program teacher collaborates with program staff and provides direct supervision and support for Transition Program teaching assistants.

• Serves as the special education teacher for assigned students in the Transition ProgramDevelops and implements IEPs including transition plans, accommodations, and goalsPlans lessons, prepares instructional materials, and presents lessons effectively in the classroom and in the communityProvides for differences of individuals and groupsEvaluates student achievement effectivelyInteracts with students in a manner that encourages respectful relationships among students and teachersDemonstrates positive interpersonal relations with staffWorks cooperatively with school personnel (Directors, Student Services staff, Deans) to benefit studentsCommunicates effectively with parents to enhance the learning process by responding to or initiating parent contacts in a timely, professional and constructive mannerExecutes Board of Education and building policies and proceduresUses appropriate channels to resolve concerns and problems

Qualifications:

• Illinois Professional Educator License (PEL) with a Learning Behavior Specialist I endorsementKnowledgeable of state and local resources available at the Transition and post-22 level for students and families
