I hereby release and discharge Quincy School District and those prior employers or other references who provide information from any and all liability as a result of furnishing and receiving this information. This information includes the sources cited above and specific examples as follows: the local Washington State Patrol, information from the Federal Bureau of Investigation of either data on all criminal convictions, or certification that no date on criminal convictions are maintained, information from SPI, the Washington or other State Department of Social Services Child Protective Services Unit and any locality to which they may refer for release of information pertaining to any findings of child abuse or neglect investigations involving me. I further agree that if I am employed, I will provide verification of my certification, education, and experience. I agree that information provided by an individual shall be confidential and I shall not have access to such information. I agree that if I have made any omission or have provided false or incomplete statements, the district may, at its sole discretion, without notice or due process procedures, terminate my employment contract. If such action is taken by the district, the contract shall be deemed void from its inception.
  5. If offered a position: * The Immigration Reform and Control Act of 1986 requires us to verify your identity and authorization to work before you may commence employment. * Pursuant to RCW Chapter 43.43, as amended, you will be required to complete a disclosure form indicating whether you have been convicted of certain crimes against persons and/or civil and administrative adjudications of assault, abuse, or exploitation. * Pursuant to RCW28A.400.301, you will be required to complete sexual misconduct disclosure forms for your previous school district employers in the United States and foreign countries. * A criminal conviction record, based on fingerprints, will also be requested from the Washington State Patrol and Federal Bureau of Investigation. The cost of such background check, including fingerprinting, will be borne by the prospective employee. I also understand and agree that I may be conditionally employed while the district performs a background record check or while the district awaits the Board of Directors making a final hiring decision as to whether or not I will be employed by the district. I understand that my employment is conditioned on the completion of both the above acts and until such time as they are completed, my employment shall only be as a casual day-to-day employee and will not in any way bind or require the district to continue my employment.

  7. Describe your philosophy of coaching (Explain your philosophy as it applies to this area)
  8. Please list each of your school coaching experiences including where you coached (include city and state), sport, position held, season (Fall, Winter, Spring), year, and if it was paid or unpaid (i.e. Cascade High School, Everett, WA, Football, Head Coach, Fall, 2007, paid).
  9. Briefly describe your level of experience and knowledge about these standards. Fostering motivation, cooperation and sportsmanship in your athletes.


ADDITIONAL REQUIREMENTS AND INFORMATION:

A school coach instructs students in the fundamental skills and knowledge of a certain sport or activity. A coach must create an environment that's conducive to personal development for team members through observing and evaluating as well as emphasizing teamwork and providing guidance for each student. Along with maintaining a positive image and for the team, a school coach has other requirements that make him/her an important part of a school. To help improve individual and team development, a coach has to develop sport-specific drill and practice activities that will enhance the team's performance. This includes exercises, game plans and teaching specific training rules to each player. A coach also can be a mentor to players and peers by promoting academic achievement for players as well as motivating students to do their best in their academic studies and sports performance.
