Social Worker

Job Responsibilities:
The Social Worker will provide individual and/or group and/or family therapy to clients on caseload with a focus on issues pertinent to child's need for RTC level of care. Specific responsibilities include but are not limited to: assessing ongoing concrete service and treatment needs of client and client's family, assisting in formulation of treatment plan with focus on discharge criteria, coordinating referrals for appropriate community and agency services, participating in planning and facilitation of monthly Parent Training Sundays and annual Family Weekends, developing and facilitating group therapy sessions, attending departmental meetings including weekly supervision, Staff Meeting, Peer Consultation, and Clinical Seminars, assisting in development of client's IEP (Individualized Education Plan) annually, attending CSE meetings, and completing, in a timely manner, all reports and documentation including but not limited to, monthly progress notes, Quarterly Treatment Reviews, CANS, Initial Psychosocial Summaries, Referral Summaries, Notifications and Discharge paperwork. This position requires 2 evening shifts a week (11am-7pm) and 1 weekend day per month.
Job Qualifications:
Must be LMSW eligible and have a valid and clean driver's license. Must possess competency in individual, group and family therapy and have the ability to travel independently. Experience with Animal Assisted Therapy/Activity interventions preferred.
