Job Summary
Works in a supervisory capacity as part of the Support Team for Educational Partnership (STEP) behavior/climate or tjr Transition Support Center Team to ensure quality clinical services for all students. Works within the school's Mutli-Tiered System of Support framework or the Transition Center Team to address the social and emotional social service needs and/or behavioral health, crisis intervention and academic needs to help stabilize the student. Provides clinical as well as support services to students, families and school personnel to promote and support students' emotional wellness to bolster academic, retention and social success or to promote successful re-entry. Serves as the link between the home, school and community. Provides behavior, social, emotional and academic intervention planning and referral services and access to resources to support students and families. Follow all State Medicaid or School District of Philadelphia guidelines for quality of service and documentation purposes.
Essential Functions
Transition Specialty
As directed by the Office of Student Rights and Responsibilities:
  • Ensures transition activities, including but not limited to supervision of case managers on the transition team, intervention documentation and oversight, and service delivery are executed in a timely manner.
  • Gathers, identifies and integrates psychological, social, cultural, medical, developmental, educational and other relevant information into the case management plan for youth; facilitates the school's identification and engagement of social and behavioral health services and works cooperatively with the School Counselor and assists with obtaining necessary supports for students.
  • Coordinates services with other agencies and resources to address the youth's needs including: education, behavioral health, medical/physical health, and social well-being; ensures youth are accessing community resources; makes collateral contacts on behalf of students as needed including child welfare workers, juvenile justice personnel, parents and caregivers, and other providers.
  • Convenes planning meetings with adults in the youth's school, family and support system to ensure a team approach for ongoing planning and intervention.
  • Complies with policies, procedures, and deadlines related to record keeping, paperwork, and documentation.
  • Ensures crisis intervention and stabilization when appropriate.
  • Provides services in the community at the youths' schools and other agencies and travels locally.
  • Provides consultation and coaching to staff around student needs.
  • Collaborates with District staff to develop and provide training to school-based staff on best practices in working with transitioning youth.
  • Participates in District and program meetings and professional development trainings as required.
  • Complies with requirements pertaining to supervision, consultation and case conference attendance.
  • Completes additional tasks as requested by the Deputy Chief.
Minimum Requirements
Transition Specialty
  • Master's degree program from an accredited college or university in social work, social services, social work administration, counseling or a related field.
  • Four years of full-time, paid, professional experience providing clinical or social services to high-risk or transitioning youth in an urban environment, three of which have been in a supervisory capacity.

Demonstrated knowledge of:
  • the principles, practices and techniques of strength-based clinical, counseling or social work for youth who are adjudicated, in foster care, or in congregate care.
  • the principles of child and adolescent development and impact of trauma and chemical dependency.
  • the principles and methods of counseling, casework and group work.
  • the functions and resources of public and private social welfare and related agencies.
  • current social, economic, and health issues, especially as related to disadvantaged youth.
  • school-based programs.
  • cultural competence and issues of race, class, and ethnicity.
  • supervisory methods and techniques.

Ability to :
  • establish and maintain effective working relationships with youth, associates, teachers, parents, community groups and social services agencies.
  • work effectively with high risk youth who have complex histories, trauma, criminal or mental health concerns.
  • analyze and interpret case records and recommend logical, sound courses of action.
  • maintain records, and prepare reports and related correspondence.
  • present ideas effectively, both orally and in writing.
  • travel locally.

Additional Job Information: This is a non-represented role. The salary grade for this position is 4026. Interviews may be conducted to determine how well a candidate meets the established criteria. Note that not all qualified candidates may be selected for an interview. Additional factors including the ability to follow directions, punctuality and professional conduct will be considered in part of the evaluation process. The selected candidate must have no anecdotal record on file and have not been subject to any disciplinary action within the past eighteen (18) months, and have an excellent record of attendance and punctuality during the last three (3) years (defined as no more than eighteen (18) occasions of lateness or absence for personal illness in the last three years (3) years with the exception of one extended long-term absence.
