Case Manager Social Worker

Under limited supervision, coordinates and directs an after-school program for at-risk students. Creates programs and activities to increase students' potential for academic and social development. Provides individual counseling and visits homes to assess environmental influences on students. Manages project budget. Reports to the principal as assigned.

Specific Duties and Responsibilities
Essential Functions:
Plans, coordinates and implements an after-school program for at-risk students creates programs and activities to increase students' potential for academic and social development; evaluates programs and makes decisions and recommendations for modifications or enhancements as necessary.

Provides direct social casework and counseling services to individual students to help alleviate any presenting problems or issues affecting students' abilities to maximize their academic and social development.

Meets with groups of students to improve peer relationships and positive attitudes toward school.

Assesses and reports on students' behavior and progress.

Makes home visits to gather data, relieve school-home difficulties and increase parents' understanding of children's educational needs and the resources available to them within the school.

Works cooperatively with school and community agencies personnel regarding the needs of particular students; consults with other disciplines on student issues and assists in locating resources to meet the environmental needs of students and their families; makes appropriate referrals.

Supervises clerical staff, interns and volunteers; supervisory duties include instructing; assigning, reviewing and planning work; maintaining standards; coordinating activities; allocating personnel; selecting new employees and recommending employee discharge and discipline as appropriate; reviews subordinates' work for completeness and accuracy; offers advice and assistance as needed.

Manages project expenditures; ensures compliance with budget allocations.

Keeps abreast of developments and innovations in the profession; attends workshops and conferences as appropriate.

Attends meetings as required.

Receives, reviews, prepares and/or submits various records and reports including case records, referral forms, ISP form, quarterly reports, evaluations, mentor monitoring reports, progress notes, questionnaires, budget documents, memos, correspondence, etc.

Interacts and communicates with various groups and individuals such as the immediate supervisor, subordinates, volunteers, interns, co-workers, District/school administrators, principals, teachers, students, parents, other District personnel, community service agencies, the Department of Social Services and other government agencies, school psychologists, social workers, consultants, medical personnel, professional peers, business and community leaders, etc.

Complies with all federal laws, state statutes, Board policies, administrative rules, and directives.

Additional Duties:
Conducts basic community and public relations tasks as required.

Performs related duties as required.

MINIMUM TRAINING AND EXPERIENCE
Requires a Master's degree in social work supplemented by six years of experience working with at-risk /adolescent populations, or an equivalent combination of education, training and experience that provides the required knowledge, skills and abilities.
