Purpose Statement

The Springfield, Missouri Public School District is currently seeking qualified applicants for the position of Transition Case Manager at Greene County Juvenile Detention. The Springfield School District is Missouri's largest public school district and has built a reputation of academic excellence based on the outstanding performance of our

students and schools. About 24,000 students attend 36 elementary schools, an intermediate school, 9 middle schools, 5 high schools and a variety of alternative education and choice programs.

The Transition Case Manager will provide support to students transitioning into and out of Greene County Juvenile Detention. The Transition Case Manager will have the specific responsibility for developing a transition plan for each student in collaboration with coordinating SPS home schools.

Essential Functions

Facilitate transition meetings before a student returns to SPS from Juvenile Detention for the purpose of ensuring all parties have communicated the best transition possible for each student..

Coordinate services with the students' case managers, juvenile officer, etc. for the purpose of discussing program or placement.

Collaborate with review officers, the home school, and the programs case manager for the purpose of seamless transition for the students and home school.

Determine what course work would be best for the student to work on while the student is in detention by coordinating with the home school for the purpose of maintaining each student's coursework and credits.

Facilitate a transition plan, including the best timeline for the return of the student to the home school for the purpose of ensuring students can return to the home school with a full schedule.

Coordinate between Juvenile Detention and the home schools for the purpose of determining what is in the best interest of the student for transition.

Facilitate the transfer of student work from home schools to Juvenile Detention, and transfer completed work back to the home school for the purpose of students being able to maintain classwork and receive credits.

Performs other related duties, as assigned.

Job Requirements (Minimum Qualifications) Knowledge, Skills, and Ability

Knowledge is required to work with home schools and understand how to make a transition plan for each student that best fits the student's needs and allows the home school input into the transition. Knowledge of confidentiality in working with students at the detention facility as well as classwork and assessment data.

Skills are required to perform multiple highly complex, technical tasks with a potential to upgrade skills in order to meet changing job conditions. Specific skill based competencies required to satisfactorily perform the functions of the job include: applying assessment instruments; operating standard office equipment; and using technology

effectively to implement instruction and to manage documentation requirements.

Ability is required to schedule activities, meetings and/or events; gather, collate and/or classify data; and use job-related equipment. Flexibility is required to work with others in a wide variety of circumstances; work with data utilizing defined but different processes; and operate equipment using standardized methods.

Ability is also required to work with a diversity of individuals and/or groups; work with a variety of data; and utilize job-related equipment. Problem solving is required to analyze issues and create action plans.

Problem solving with data frequently requires independent interpretation of guidelines; and problem solving with equipment is moderate.
