Description

Summary: Develop annual transition plans for all students age 14 and older in accordance with local, state and federal guidelines. Provide support and guidance to students and their families with linkages to federal, state and local resources and connections with post-secondary education, training programs, and/or adult service providers. Provide resources and support to staff in the areas of career development, independent living, community participation, self-advocacy and self-determination.

Qualifications
  • Minimum of BA degree (graduate degree preferred) in special education or a human service related field
  • 3-5 years' work experience in special education or related field
  • Knowledge of transition planning process and ability to develop transition plans in compliance with Indicator 13
  • Knowledge of post-secondary education/training programs, state and local disability support resources, and adult service providers in the greater metropolitan area
  • Ability to effectively communicate and work with students and families from all backgrounds
  • Ability to effectively collaborate with staff and IEP team members
  • Ability to manage multiple tasks simultaneously and meet deadlines
  • Demonstrate computer skills in word processing and other software programs as needed
  • Excellent oral and written communication skills

Responsibilities
  • Conduct age appropriate transition assessments based on identified post-secondary outcome goals
  • Collaborate with student, staff, and family in developing annual transition services/activities and IEP goals that are reflective of the transition assessment
  • Assist students/families in establishing connections with the appropriate community resources/agencies based on anticipated service needs
  • Collaborate with program coordinators to identify curriculum that will assist students in acquiring strategies to become more independent and self-directed
  • Support and promote student participation and engagement during the IEP meeting
  • Conduct training for staff on federal, state and local transition policies and procedures related to the development of annual transition IEP goals
  • Become familiar with local disability services and community resources

Ivymount School and Programs provides school- and community-based services for children and young adults with special needs from throughout the Washington metropolitan area. The school program serves more than 200 students, ages 4 – 21, with speech and language impairments, learning disabilities, intellectual disabilities, health impairments, and Autism Spectrum Disorders. The classrooms are structured to serve students with a range of academic and cognitive abilities, including, in some cases, those who have the potential to obtain a high school diploma. Ivymount Outreach Programs serve students, teachers, and parents to support children with learning and developmental challenges in the community through assessments, evaluations, therapies and professional development. The Maddux School is preschool-2nd grade program that combines creative approaches with traditional early childhood education in a unique transdisciplinary model. The goal in all of the programs is to help children and young adults achieve their highest level of development and independence and to live with dignity and self-respect. Ivymount School has been twice recognized as a Blue Ribbon School of Excellence by the U.S. Department of Education. Founded in 1961, Ivymount is celebrating its 60th year!
