JOB TITLE: School Community Care Coordinator

REPORTS TO : Chief of Youth, Family, and Community Engagement

NATURE OF WORK
Mid-level management and professional position requiring a wide variety of skills relating to the New Haven students and families. The School Community Care Coordinator focuses explicitly on families dealing with severe crises that affect the engagement of students in school. The School Community Care Coordinator works with a caseload of youth and families to identify the root causes and underlying family needs, builds a safety net of supports, and gradually releases families from the caseload. The Care Coordination approach is a specific strengths-based, culturally affirming intervention aimed at supporting at-risk children, youth, and families to achieve optimal levels of functioning in school, at home, in the community and the workplace. The vision driving this model is to build healthy youth and strong communities by strengthening the linkages between schools and communities to support children and families' abilities to meet life's challenges. Given the stresses and strains associated with modern-day realities, particularly the long-term impact of a worldwide pandemic, increased influence of social-media and steady spikes of community violence, school-community care coordination is intended to foster youth and family resiliency.
ILLUSTRATIVE EXAMPLES OF WORK
  • Collaborates with schools to determine eligibility for Care-Coordination Services.Completes required care-coordination certifications.Conducts regular home visits with each family in crisis and establish rapport with members and community-based providers to meet student needs. Collaborates with schools to collaborate and participate in an interdisciplinary school team.Works closely with community partners in Greater New Haven to link multiple services, healthcare providers, and community resources to meet families' needs.Conducts Child and Family Team meeting (CFT), where the family and multiple providers meet to create and/or continuously refine a written plan of care; leads family-youth collaborative meeting.Emphasizes prevention and early restorative intervention to create safe learning environments.Links families to sustainable resources and collaborates with other providers working with families in order to provide wrap around services. Provides professional development, training, and presentations on care coordination to all levels of school district community members (school sites, parents, students, community partners).Models problem-solving strategies with teaching staff and student support service teams.Participates in professional development with community-based services providers, such as Clifford Beers and others, and participates in conferences and other opportunities to enhance care coordination skills.Attends meetingsMaintains reports and updates regarding the implementation status at school sites.Collaborates and assists with data collection and analysis activities to demonstrate initiative progress and effectiveness.

REQUIREMENTS OF WORK
  • Graduation from an accredited college, university, or three to five years of experience in community based work. Qualifying knowledge level of community-based work that will facilitate the successful implementation of position's tasks.Knowledge of working with youth and families.Knowledge in program development, coordination, and evaluation.Knowledge and experience in providing professional development and presentations.Excellent planning and organizational skills with a proven ability to execute tasks efficiently.Strong communication and facilitation skills, including clear and persuasive speaking and writing.Ability to demonstrate strong, compassionate, inclusive leadership.Ability to work effectively with a wide variety of people.Deep commitment to young people and the values and principles restorative practices.Demonstrate an understanding of adolescent development (emotional, social, physical, and intellectual)Deep knowledge of and commitment to social justice and equity.Must have a valid driver's license and reliable transportation.

**This is a grant-funded position by ESSER II Grant, with an end date of June 30, 2023. New Haven Public School District anticipates this position will not continue beyond June 30, 2023.
