Overview

An employee who is responsible for managing the Transition School to Work (TSW) program/contract for the Local Education Agency (LEA). This person is self-directed and coordinates activities with the Pre-ets Specialist and the Special Education Supervisor/designated school contact.

Responsibilities

  • Serve as the point person for the Transition School to Work (TSW) grant.
  • Receive referrals from LEA and inform VR Counselor. Assist VR Counselor in completing application. Obtain necessary documentation from LEA for eligibility determination. Assist in scheduling and coordinating needed evaluations to assist in the VR eligibility process as directed by the VR Counselor.
  • Review goals and priorities as identified in the TSW contract with LEA management and Pre-Ets specialist or supervisor.
  • Coordinate with students and school personnel to reach pre-employment goals set forth in the IEP and IPE (if the student becomes an eligible client for Vocational Rehabilitation).
  • Provide guidance to LEA personnel on how to help students achieve goals and objectives.
  • Ensure that all recipients records are properly maintained, stored and kept confidential at all times.
  • Lead the TSW/LEA team to select appropriate curricula for working with students. Implement appropriate curricula or demonstrate creativity in developing programs/training to assist personnel. Seek assistance from the Pre-Ets Specialist as needed.
  • Provide guidance to grant/LEA personnel on how to help students/recipients receive the one or more of the five identified services in order to achieve goals and objectives.
  • Schedule and attend events and training with Pre-Ets Specialist, VR Counselor, and other TSW grant employees to further ensure student goals are met.
  • Notify Pre-Ets Specialist/Supervisor of upcoming IEP meetings. Attend IEP meetings to report student progress and to explain the process for referral to VR for IEP team members.
  • If the program is receiving services from a Community Rehabilitation Provider (CRP), ensure that there is no duplication of services provided to the students.
  • Handle student, work site, and/or LEA complaints and work to resolve the problem. Seek guidance from Pre-Ets Specialist/Supervisor as needed.
  • Assist students (and their families) in completing necessary documentation for post-secondary education application and financial aid assistance.
  • Serve as a liaison to post-secondary educations' Disability Services offices.
  • Assist the Project SEARCH team by recommending and participating in the student selection process to complete the year-long internship. Make sure the VR Counselor has all necessary materials/documents to complete the eligibility process.
  • Maintain a high level of confidentiality, a professional demeanor, and represent both organizations in a positive manner at all times
  • Ensure all Pre-Ets monthly reporting is completed accurately on the forms provided and send to designated personnel timely each month. Receive referrals from LEA personnel and inform Pre-Ets Specialist/Supervisor of any new VR referrals.
  • Document student progress and complete other documentation necessary for student's future employment. Provide documentation to LEA and VR Counselor.
  • Understand and keep up to date with new technology, community resources, and assistive devices available to individuals with disabilities.
  • Meet monthly with the Project SEARCH team to ensure fidelity of the program.
  • Uses technology to enter and access information related to essential job functions.
  • May operate a motor vehicle to conduct various work activities.
  • Must be willing to travel to various locations as needed.
  • Performs other duties as assigned.

Qualifications

Knowledge/Education/Experience:
  • B.S./B.A. in education or human service related field
  • Two years' experience in teaching or services to individuals with disabilities
  • Knowledge of federal, state, and local laws, rules, regulations, policies and procedures relating to students with disabilities. Or, any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job.
  • Must maintain a valid driver's license due to frequent travel to schools.

Skills/Qualifications:
  • Demonstrate excellent verbal and written communication skills to deal effectively with students, educators, Board members, parents, and community members.


Compensation :
Salary is based on a combination of relevant education and experience.
