ONE (1) TRANSITION COORDINATOR

Qualifications:

  • Must hold a New Jersey Certificate of Teacher of the Handicapped, or Teacher of Students with Disabilities with endorsement, School Psychologist, Social Worker, Administrative Certification
  • Bachelor's degree from an accredited university, Master's preferred
  • Certified in Structured Learning Environment (SLE) preferred
  • OSHA Certified Preferred
  • Knowledge of special education laws, procedures, and protocols as per N.J.A.C 6A:14
  • Must be able to develop partnerships and relations with external agencies to support students with disabilities ages 18-21
  • Must be able to identify transition services provided by state and community agencies; DDD, DVRS, SSI, PerformCare, CMO, etc.
  • Must be able to coordinate school and community work based learning
  • Must be able to participate in transitional IEP meetings for consultation to Child Study Teams in the establishment and implementation of transitional goals
  • Coordinate and develop career awareness and exploration activities
  • Attend and participate in school events and activities
  • Coordinate necessary transportation services for school/field activities
  • Provide professional development with teachers and staff
  • Excellent interpersonal skills, integrity, and moral character
  • Ability to work collaboratively with administrators, staff, and community
  • Ability to communicate effectively in English, both orally and in writing, with students, staff, parents, administrators, and the community
