PRIMARY FUNCTION: This position is responsible for assisting with the transition of students back to their zoned school after long-term or expulsion.

REPORTS TO: Principal, Building Bridges Academy
SALARY SCHEDULE: A-5
WORK DAYS: 229

REQUIREMENTS:
  1. Education Level: Bachelors Degree in Social Work, Counseling, Sociology or Psychology from an accredited college or university
Preferred : Masters Degree in Social Work, Counseling, Sociology or Psychology from an accredited college or university

Experience, Skill, and Certification:
  • At least 5 years of directly related work experience
  • Demonstrated ability to build successful relationships with children, families, teachers and the community
  • Demonstrable knowledge of community resources and the ability to make effective referrals for families in need
  • Demonstrable ability to assess the needs of students assigned to the program
  • Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate.
  • Excellent organizational and time management skills
  • Excellent oral and written communication skills, to include speaking with parents and others in person and by phone;
  • Problem solving skills and ability to multi-task
  • Successful experience working with diverse populations
  • Able to travel between schools, students homes and community organizations
  • Demonstrable skill with Microsoft Office suite of software products
Duties and Responsibilities:
  1. Provide holistic and comprehensive case management services to students to include intake assessment, goal setting, long-term case plan development, monthly case plan development, progress monitoring, advocacy and referrals.
  2. Connect families/students with needed and available community resources.
  3. Communicates with principals, teachers and school staff regarding the individual needs of students and makes appropriate referrals.
  4. Maintains up-to-date student records and progress notes on students served.
  5. Works with principals, teachers and parents/guardians and students to develop a transition plan for students transitioning out of the program.
  6. Provides follow-up services and maintains monthly reports on identified students during the transition period.
  7. Assists with the implementation of supplemental educational services for students in alternative school setting.
  8. Communicates with parents on an ongoing basis regarding attendance, behavior and academic resources.
  9. Participate in regular staff meetings, staff training programs, supervisory sessions, and accept the responsibility for aiding the development of positive team relationships as requested.
  10. Performs other duties as necessary for the effectiveness of the organization and the safety of students.
TERMS OF EMPLOYMENT
Incumbents will be considered "at will." Appropriate pay will be determined based on the Grade as determined by Human Resources and allowable experience. The work calendar will be 229 days. (A-5) Exempt.
