Job Summary
The Coordinator, Transition and Social Services acts as an outreach person to families in order to provide assistance in the areas of referral to social service and health agencies, family and student counseling, and the implementation of site-based student and parent programs. The coordinator assists in the planning and implementation of strategies to reduce absenteeism, to initiate academic engagement, and to guide appropriate social interactions. The position has a retirement association with the Public Employees' Retirement System (PERS).

Job Requirements
The following are required:
  1. A bachelor's degree in social services, counseling, health, or a related field.Three or more years of experience in social services.Minimum of three years of experience working within a community agency.Experience in working with bicultural and bilingual families.Must be familiar with educational, cultural, health, and social services resources for all students, no matter their ethnic background or gender.Ability to establish and maintain communication with relevant community organizations.Ability to coordinate meetings and activities.Knowledge of record keeping and ability to prepare reports.Ability to establish and maintain productive working relationships with others.Computer and organizational skills.Crisis prevention and intervention skills.Ability to maintain confidentiality.Personal transportation is required for home visits, community outreach, traveling between programs and meeting attendance. Mileage is reimbursed as outlined in the ACE negotiated agreement or commiserate with positions that are itinerant.

The following are preferred:
  1. A bachelor's degree with an emphasis in social services.Knowledge of challenges facing youth and families from low socioeconomic standing.Knowledge of community resources available to support youth and families.Ability to relate to parents, students, teachers, and administrators in a supportive and professional manner.

Essential Job Functions
  1. Responds to parent, student, teacher, and staff requests for assistance with social service related needs.Coordinates with school counselors, school nurses, principals, Child in Transition staff, and other school personnel to identify and address social services related needs of individual students.Consults and collaborates with community agencies, organizations and Anchorage School District interdisciplinary teams to maximize educational opportunities for students. Makes social service referrals when appropriate and assists with developing action plans.Works with youth and families to improve school attendance.Assists in the development of program needs.Confers with families referred by teachers/staff on an as-needed basis regarding academic needs, attendance, health, and social service needs of students. Addresses family-school issues through home visits, school visits, phone calls, letters, or e-mail.Coordinates student transition plans with school counselors and establishes a routine for periodic check-ins as needed.Provides limited therapeutic counseling for students in crisis and initiates appropriate services to support the students as needed.

Physical/Mental Demands
The physical demands of this job require frequent standing, walking, sitting, speaking, and hearing. Specific visual abilities are also required. The employee is regularly required to reach with his/her hands and arms and occasionally lift items weighing less than 40 lbs. Additionally, the employee must be able to communicate by oral and written means in an appropriate business manner and have cognitive skills to understand instructions, readily recall facts and details, handle conflict, and make effective decisions under pressure. The employee must have the ability to effectively manage the stress of working with students, parents, and other employees representing diverse cultures, personalities, and work styles in a dynamic work environment.

Work is performed in a professional environment with a wide variety of individuals having differing functions, personalities and abilities, including working with diverse groups of people in a variety of different settings.

While performing the duties of this job the employee may be regularly exposed to a video display. The employee may be exposed to outdoor weather conditions. The noise level in the work environment is usually moderate.
