Devereux Advanced Behavioral Health, Brandywine School provides programs and services for children and adolescent males who experience behavioral and psychiatric challenges. Located in Glenmoore,PA. We are currently seeking an Educational Transition Coordinator to work in our Approved Private School with the following responsibilities:

Collaborates with the interdisciplinary team on each client to ensure full understanding and educational status of each assigned client.

Attends Individualized Education Plan (IEP) meetings as needed to ensure an uninterrupted school placement post discharge.

Schedules educational transition meetings as needed. Coordinates the job posting, interview, paperwork and follow-up for the employment of client workers.

Develops, implements and monitors systems designed to facilitate successful transitions of Pennsylvania clients in admission to and discharge from Center educational programs.

Maintains communication with school districts and families/guardians on issues related to the transition at discharge.

Ensures that educational information on each client is available and forwarded to the new school prior to discharge.

Maintains up-to-date knowledge of Center programs and current developments in the behavioral healthcare and education fields to ensure accurate information sharing with internal and external customers.

Develops and utilizes processes to evaluate the efficacy of transition framework.

Qualifications

Education & Experience: Bachelors degree. Minimum of two years experience within behavioral healthcare strongly preferred. Experience and skill demonstration may be considered in lieu of formal degree.

Physical Requirements: Significant amounts of walking, sitting, and standing. Some lifting and/or managing resistance required related to applying physical intervention techniques - upwards of 75-100 lbs. or more. Some repetitive wrist/hand movements, normal hearing and vision. Occasional use of computers and office equipment. Occasional driving of Devereux vehicles for business purposes. Ability to meet 100% mastery of Safe and Positive Approaches (S&PA) techniques.

Devereux requires all individuals interviewing for a position involving contact with children to complete The Diana Screen, a screening that assesses a candidate's understanding of appropriate boundaries between adults and children

Work Environment: Regularly works inside areas that are adequately lighted and ventilated. Some fluctuations in temperature. Protective equipment - latex gloves, facial mask protective eyewear, gown. Possible exposure to blood-borne pathogens. Physical involvement in exercise of S&PA techniques (when required).

Core Curriculum: As required annually by licensing agencies and Devereux (organization and Center-specific modules).
