    This is a probationary position only for any external candidate.     Located just seven miles north of downtown Denver, Adams 12 Five Star Schools is close to the conveniences of a major city center yet far enough to offer the quiet of the suburbs. The five stars represent the five unique communities the district serves: Broomfield, Federal Heights, Northglenn, Thornton and Westminster.

    SUMMARY: A transition specialist will assist students (18-21 years old) with disabilities and their families in obtaining services necessary to transition to a post-secondary setting, whether for post-secondary education, independent living skills, employment, or long-term support.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:
    - Plan, prepare and deliver instructional activities in the school classroom and in community settings.
    - Disseminate information and collaborate with staff, students, parents, and district high school special education teams regarding student needs, progress, and resources
    - Manage IEPs and compliance with IDEA for assigned caseload: coordinate and facilitate IEP meetings, write IEPs, and communicate with support staff.
    - Facilitate referral, application, eligibility process for students and families to adult service providers and community organizations.
    - Collaborate with work sites to ensure safety of students and access to high-quality learning opportunities
    - Provide direct care services for students including: toileting/restroom support; managing seizure care- administering medication, calling 911 and parents, and managing the emergency personnel; Tube feeding, Daily medication, Sensory diets, Support when eating, Helping students transfer to PT equipment.
    - Manage student behavior in the classroom and community creating appropriate behavior strategies (including significant behaviors from students with Autism)
    - Create schedules and observe/provide feedback to transition coaches providing support to students
    - Create a positive educational climate for students to learn in.
    - Participate in ongoing training sessions.
    - Create lesson plans and modify accordingly throughout the year.
    - Read and stay abreast of current topics in special education.
    - Utilize various curriculum resources and technologies to enhance student learning.
    - Integrate competencies, goals, and objectives into lesson plans.
    - Establish and communicate clear objectives for all learning activities.
    - Observe and evaluate student performance.
    - Train staff on communication devices
    - Perform other related duties as assigned

    EDUCATION AND RELATED WORK EXPERIENCE:
    - Master's degree or higher in special education.
    - Must be a minimum of 18 years old.
    - Minimum of 3 years teaching experience working with students with disabilities preferred
    - Understanding of disability areas under IDEA.
    - Knowledge of how disabilities impact an individual's ability to function in the post-secondary setting.
    - Knowledge of state and federal laws/regulations that protect individuals with disabilities in education, employment, and community.

    LICENSES, REGISTRATIONS or CERTIFICATIONS:
    - Criminal background check required for hire.
    - Must possess, or be able to obtain from, or be in process with, the Colorado Department of Education (CDE) for a valid Colorado Educator License within 30 days of hire.
    - Must possess the required license and endorsement. If highly qualified by content test or semester hours, with no endorsement: applicant must possess a Colorado educator license with a passing score on a CDE approved content test or 24 semester hours in specific core content classes as determined by Human Resources.
    Preference given to candidates with ELL Training and/or an ESL or (LDE) Linguistically Diverse Education Endorsement.

    SALARY INFORMATION (FOR EXTERNAL CANDIDATES):
    -The salary listed is for full time positions (1.0 FTE). This salary will be adjusted, as needed, based upon the FTE.
    -The salary grade posted is the starting salary range for Bachelor's Step 1. Salary for position may vary based on education and experience.
    -Salary placement on our Certified Salary schedule is dependent on the employees' education and/or work experience. Initial placement will be at bachelor (BA) step 1. If minimum qualifications are higher than a (BA), they will be placed at the initial requirement level for this position.
    -After initial placement by the district on the salary schedule, the certified/licensed employee can provide the district additional official transcripts and/or documentation to support further salary advancement.
    -Certified/licensed employees will be granted one (1) year for each full year of contractual and/or work experience up to a maximum of six (6) years, which includes in-district and out-of-district contractual teaching and/or work experience. Responsibility for verification of outside experience shall rest with the individual employee.
